All Eagle Mount participants are required to submit an Annual Participant Application.
You will be contacted by Eagle Mount staff to confirm your enrollment in our camps and programs in advance of each session and when your application is about to expire. If you have not heard from program staff by two weeks in advance of a camp or program, or for more information, contact us at (406) 586-1781 or via email.

In addition to the Annual Participant Application, a PHYSICIAN APPROVAL FORM must be completed and signed by each participant’s physician. This form will be auto-generated and emailed to you at the end of the application.
